Weddings: important people & group photos

Please use this form to give me details of the important people who will be at your wedding (close family, bridesmaids, groomsmen etc.) and also any specific formal group photos that you would like me to take.

  • Please gives a name and relationship for each family member - e.g. "Janet - Mum", "Brian - stepDad". Usually listing just parents and siblings is sufficient - include aunts and uncles etc only if they have a specific role to play at the wedding e.g. giving the bride away or making a speech at the wedding breakfast.
  • Details please as for the bride or first partner above.
    Please tick all group shots that you would like me to take. Bear in mind that the more groups we do the longer it will take!

    If any of your parents are divorced and remarried, please let me know in the "any other comments" below if there are any issues I should be aware of regarding who is photographed with whom.

  • Please use this box to let me know of any other requirements that you may have concerning the photography.
  • It's helpful if I can have names and contact numbers (mobile phone) for the best man and chief bridesmaid or matron of honour, or other suitable people, in case of emergency. If you have a wedding planner I would appreciate having his or her number as well.
